How do you have auto follow enabled?
There's basically two ways:
Settings tab: [x] Auto Follow Mode
Setup tab: Auto Follow [_____]
This is basically "in-game" follow. It just does /follow on that person when appropriate. It turns OFF follow for a multitude of reasons, such as being in combat and having to face an NPC. It has to disable this follow or it would keep turning back. It doesn't turn it back on until combat is finished.
The superior way, IMO, is to use OgreFollow (OFol).
You make an MCP button, and use it. This follow will continue through combat, unless people have move into melee range/move behind/move in front checked. Since it doesn't force you to keep turning back.
Based on your "works" "not works", I'd guess you're using the first method.
