Looking for some assistance with a monk.
I'm trying to get the combination ability to fire. It does so some times, but not always. Here are parts of log files and I'm wondering if someone can advise me why it's not working.

Best I could tell from there, it's a proc, or a DoT.
Here's proof that those 3 abilities do indeed work...

And here is no procs or DoTs (with the exception of Dragon Stance which is in all the working combinations).

So my question is... Anyone have any idea why my combinations aren't working all the time?
I've tried to test the following:
Doing all 3 in 2s or less = works some times, not others.
Doing all 3 in 4s or so = works some times, but not others.
Not doing DoTs.
Not having procs.
I'm basically stuck to why combinations aren't firing. If you need more of a log file let me know, but I my understanding is casting those 3 abilities within a short time period should = combination.
