Can someone give me an exact example of where this is happening? IE: Give me a zone name and a /loc, or some general way of finding it. I have a few tricks for finding things like this.
The same thing is also true for the entrance to Wurmbone's End in JW. ISXEQ2 just doesn't seem to see it as anything, not even an unknown.
It is a "NoKill NPC" and works "fine" ish. The only part that doesn't work, and unknown to me if it's EQ2 or ISXEQ2, many many "doors"/Zone objects do not appear where you think they would be.
For example, many of them appear at location 0,0,0. I'm unaware of if they are actually at 0,0,0, but visually appear there, or if ISXEQ2 just can't read the location, so it reports as 0.
I'm inclined to believe it's ISXEQ2, because the distance to click these objects still exists (IE: can only click when 10m or less from them in game).
Amadeus, if you want to see an example do the following:
Use a bell and go to: Jarsath Wastes
Turn around and there is an NPC: Brind
Hail him, then select the second option: "Actually, just send me to Wurmbone Cleft"
Then select the first option: "Thank you!"
Then go into the cave and it's the door on the left.

There are no NPCs around here, so any level of character should be good to try it.
